2. SERVICE AUTHORIZATION
General: I/We hereby grant full permission to Angel Paws USA to carry out all necessary actions related to the pet funeral services. This includes transportation, cremation, and any other services specified in the selected package. Cremation: In the event that the Cremation Service is chosen, I expressly authorize the removal and cremation of the pet. Euthanasia: If the Euthanasia service is selected, I grant permission for the euthanasia procedure, followed by the removal and cremation of the pet. Burial: For Burial services, I authorize the removal and subsequent burial of the pet. Autopsy/Necropsy: If an autopsy or necropsy is opted for, I authorize a post-mortem evaluation of the Decedent Pet.
General Transport:
I permit Angel Paws USA, in conjunction with their partners, to oversee or aid in transporting the remains, be it to/from my home, an autopsy facility, or a crematory.
Documentation: I commit to furnishing or aiding Angel Paws USA and their partners in obtaining any vital documentation to thoroughly probe the cause of death.

5. RABIES EXPOSURE DECLARATION
Exposure Status: To the best of my knowledge, the pet has not bitten, scratched, or potentially exposed any individual or entity to rabies within the past ten (10) days. Rabies Test: If the pet has bitten or may have exposed someone within the specified timeframe, I understand that a rabies test is necessary.

Understanding: I acknowledge euthanasia as a compassionate measure to end an animal's life and alleviate unnecessary suffering.
Accuracy: I guarantee that all information provided by me is accurate and comprehensive.
Immediate Execution: Upon endorsing this agreement, I understand that my stated wishes may be carried out promptly.
7. TRANSPORTATION & VEHICLE SELECTION
General Understanding: I/We understand that Angel Paws USA has the discretion to choose the vehicle type when transporting my pet from the pick-up spot to their facilities. The selection may include cars, vans, SUVs, or pickups, depending on logistical needs such as the pet's size and weight. As the owner, I/we have no authority over the vehicle choice.
Multiple Pet Transport: I/We acknowledge that Angel Paws USA may transport multiple pets at the same time to optimize efficiency. However, I/We can be assured that every pet will be treated with dignity and care throughout the transportation process.
Promptness Commitment: Angel Paws USA is committed to reaching the pick-up spot within an estimated span of 1-3 hours. This commitment highlights their dedication to providing timely and compassionate services during such challenging times for pet owners.
8. VARIABLE RESPONSE TIME
General Understanding: We understand that the time it takes to process removals can vary due to factors such as call volumes and unforeseen circumstances. While we strive to be prompt, there may be instances where the wait time exceeds the usual 1-3 hours. Refund Policy: If the delay is within a 12-hour window, no refunds will be issued.

Role Clarity: I/We understand that Angel Paws USA operates as a pet funeral service and has the authority to select the crematory for my pet.
Quality Assurance: Every chosen crematory undergoes a thorough evaluation to ensure they adhere to Angel Paws USA's standards of care. The company is committed to partnering with the region's best crematories, ensuring top-notch services.
10. PAYMENT AGREEMENT & TERMS DISCLOSURE:
Cremation Services Payment Process:
Deposit: A non-refundable deposit of $150 is required upon completing this form.
Balance Payment The remaining amount, including any additional selections, will be automatically charged after removal.
Payment Delays: If the payment is not processed, a $50 fee will be added if not settled within 24 hours. After 48 hours without payment, communal cremation will be executed without any refunds.

Advanced Testing: If the attending physician suggests advanced testing, the costs will be discussed before sending to external labs. Separate billing may occur for this type of testing.
Reporting:
- Preliminary Report: Delivered via phone within 48-72 hours after the gross examination.
- Final Report: Completed in 10-15 business days. Delays may occur due to special testing, with updates provided through email.

Your charges are determined by the pricing agreement that has been provided to you.
11. PACKAGE DETAILS & TIMELINES
Package Contents: Angel Paws USA reserves the right to modify the components of the package. While we strive to deliver the items as specified in our packages, there may be circumstances that require the replacement or alteration of items, especially if they are unavailable. Estimated Package Ready Time: Typically, packages are prepared within 7-10 business days, excluding weekends and public holidays. Please note that this is an approximation and not a strict deadline. Unclaimed Packages: If a package remains unclaimed for 15 days after it is ready, a storage charge of $50 will be incurred. If the package is not claimed within 30 days of notification, it will be discarded without any reimbursements.
12. DISPUTE RESOLUTION
Initial Resolution Steps: In the event of any disagreements related to this agreement, both parties should first attempt to find a solution through open and honest discussions. Mediation: If a solution cannot be reached within a month, either party may initiate mediation through written communication. The parties will mutually select a mediator. If an agreement still cannot be reached, the services of a reputable mediation organization will be sought. The mediation process will adhere to the rules established by the chosen mediation body.
Legal Action: If mediation proves unsuccessful, the next course of action would be to pursue legal remedies. Any legal actions will be exclusively subject to the courts of [Miami Dade County]. Both parties agree to submit to the jurisdiction of the [Miami Dade County] courts, waiving any objections regarding the venue.
13. INTELLECTUAL PROPERTY
Rights and Protections: All content, trademarks, logos, and other intellectual properties associated with Angel Paws USA are protected under copyright and trademark laws. Unauthorized use or replication without explicit consent is prohibited.
14. USER-GENERATED CONTRIBUTIONS
Usage Rights: Any reviews, testimonials, or photos submitted may be used by Angel Paws USA for promotional or other purposes. Contributors retain personal rights to their submissions.
15. PROHIBITED ACTIVITIES
Customer Conduct: Clients are expected to act with respect and understanding, especially during service interactions. Any behavior deemed disruptive or harmful may result in termination of services.
