1. OWNER CERTIFICATION
Ownership: I hereby certify that I am the owner or the authorized agent of the deceased pet. Disposition Rights: I confirm that I possess the legal right to control the disposition of the remains of the mentioned Decedent Pet.
2. SERVICE AUTHORIZATION
General: I/We hereby grant full permission to Angel Paws USA to carry out all necessary actions related to the pet funeral services, including transportation, cremation, and any other services specified in the selected package. Cremation: In the event that the Cremation Service is chosen, I expressly authorize the removal and cremation of the pet. Euthanasia: If the Euthanasia service is selected, I grant permission for the euthanasia procedure, followed by the removal and cremation of the pet. Burial: For Burial services, I authorize the removal and subsequent burial of the pet. Autopsy/Necropsy: If an autopsy or necropsy is opted for, I authorize a post-mortem evaluation of the Decedent Pet.
3. GENERAL TRANSPORT
Transport Permission: I permit Angel Paws USA, in conjunction with their partners, to oversee or aid in transporting the remains, be it to/from my home, an autopsy facility, or a crematory. Documentation: I commit to furnishing or aiding Angel Paws USA and their partners in obtaining any vital documentation to thoroughly probe the cause of death.
4. PARTNERS PROGRAM AUTHORIZATION
Authorization: For associated Partners Program services, I authorize cremation and/or removal as needed.
5. RABIES EXPOSURE DECLARATION
Exposure Status: To the best of my knowledge, the pet has not bitten, scratched, or potentially exposed any individual or entity to rabies within the past ten (10) days. Rabies Test: If the pet has bitten or may have exposed someone within the specified timeframe, I understand that a rabies test is necessary. In such cases, we will not handle the case as the head needs to be sent to a lab, and we typically do not handle such cases.
6. EUTHANASIA AWARENESS
Understanding: I acknowledge euthanasia as a compassionate measure to end an animal's life and alleviate unnecessary suffering. Accuracy: I guarantee that all information provided by me is accurate and comprehensive. Immediate Execution: Upon endorsing this agreement, I understand that my stated wishes may be carried out promptly. Process: The process includes administering a sedative, followed by anesthesia, and finally, a drug to stop the heart.
7. TRANSPORTATION & VEHICLE SELECTION
General Understanding: I/We understand that Angel Paws USA has the discretion to choose the vehicle type when transporting my pet from the pick-up spot to their facilities. The selection may include cars, vans, SUVs, or pickups, depending on logistical needs such as the pet's size and weight. Multiple Pet Transport: I/We acknowledge that Angel Paws USA may transport multiple pets at the same time to optimize efficiency. However, I/We can be assured that every pet will be treated with dignity and care throughout the transportation process. Promptness Commitment: Angel Paws USA is committed to reaching the pick-up spot within an estimated span of 1-3 hours. This commitment highlights their dedication to providing timely and compassionate services during such challenging times for pet owners.
8. VARIABLE RESPONSE TIME
General Understanding: We understand that the time it takes to process removals can vary due to factors such as call volumes and unforeseen circumstances. While we strive to be prompt, there may be instances where the wait time exceeds the usual 1-3 hours. Clients are always kept informed about the expected response time. In cases where we cannot pick up the pet within the same evening, clients are offered a refund.
9. CREMATORY SELECTION
Role Clarity: I/We understand that Angel Paws USA operates as a pet funeral service provider and has the authority to select the crematory for my pet. Quality Assurance: Every chosen crematory undergoes a thorough evaluation to ensure they adhere to Angel Paws USA's standards of care. The company is committed to partnering with the region's best crematories, ensuring top-notch services.
10. PAYMENT AGREEMENT & TERMS DISCLOSURE
Cremation Services Payment: A non-refundable deposit of $150 is required upon completing this form. The remaining amount, including any additional selections, will be automatically charged after removal. Euthanasia Services Payment: An initial non-refundable fee of $350 is required upon completion of the form. The remaining amount will be automatically charged when the attending doctor arrives. Necropsy/Autopsy Services Payment: Upfront payment includes transport, detailed examination, in-house testing, consultation with a certified veterinary pathologist, and a private cremation. Partners Program: Your charges are determined by the pricing agreement that has been provided to you.
11. PACKAGE DETAILS & TIMELINES
Package Contents: Angel Paws USA reserves the right to modify the components of the package. While we strive to deliver the items as specified in our packages, there may be circumstances that require the replacement or alteration of items, especially if they are unavailable. Estimated Package Ready Time: Typically, packages are prepared within 7-10 business days, excluding weekends and public holidays. This is an approximation and not a strict deadline. Unclaimed Packages: If a package remains unclaimed for 15 days after it is ready, a storage charge of $50 will be incurred. If the package is not claimed within 30 days of notification, it will be discarded without any reimbursements.
12. DISPUTE RESOLUTION
Initial Resolution Steps: In the event of any disagreements related to this agreement, both parties should first attempt to find a solution through open and honest discussions. Mediation: If a solution cannot be reached within a month, either party may initiate mediation through written communication. The parties will mutually select a mediator. If an agreement still cannot be reached, the services of a reputable mediation organization will be sought. Legal Action: If mediation proves unsuccessful, the next course of action would be to pursue legal remedies. Any legal actions will be exclusively subject to the courts of Miami-Dade County. Both parties agree to submit to the jurisdiction of the Miami-Dade County courts, waiving any objections regarding the venue.
13. INTELLECTUAL PROPERTY
Rights and Protections: All content, trademarks, logos, and other intellectual properties associated with Angel Paws USA are protected under copyright and trademark laws. Unauthorized use or replication without explicit consent is prohibited.
14. USER-GENERATED CONTRIBUTIONS
Usage Rights: Any reviews, testimonials, or photos submitted may be used by Angel Paws USA for promotional or other purposes. Contributors retain personal rights to their submissions.
15. PROHIBITED ACTIVITIES
Customer Conduct: Clients are expected to act with respect and understanding, especially during service interactions. Any behavior deemed disruptive or harmful may result in termination of services.
16. TERMINATION OF SERVICE
Service Refusal: Angel Paws USA reserves the right to refuse or terminate services under certain conditions, including non-payment, disruptive behavior, or non-compliance with terms.
17. WARRANTIES & GUARANTEES
Service Standards: Angel Paws USA adheres to industry-leading standards for all services provided. Specific quality guarantees will be communicated as appropriate.
18. ELECTRONIC COMMUNICATIONS
Communication Methods: By using our services, clients consent to receive communications from us via email, SMS, or other electronic means.
19. THIRD-PARTY LINKS OR SERVICES
External Referrals: Angel Paws USA may suggest third-party services or products. We are not responsible for the content or quality of these third-party offerings.
20. REFUND OR REDRESSAL PROCESS
Service Concerns: While all sales are final, any grievances or concerns about received services can be directed to our customer care for consideration.
21. DATA PROTECTION AND PRIVACY
Personal Data Handling: Angel Paws USA respects client privacy and handles personal data collected with utmost care, in accordance with prevailing data protection laws. Usage: Personal data is exclusively used for the facilitation of the requested services. No third-party disclosure will occur unless the client explicitly agrees or it becomes a legal necessity.

Use and Disclosure: Client data is exclusively used for the facilitation of the requested services. No third-party disclosure will occur unless the client explicitly agrees or it becomes a legal necessity.
Governing Principles: This Agreement adheres to and is interpreted in alignment with the laws of the State of Florida, overlooking any conflict of law rules. Complete Agreement: This document embodies the full accord between the involved Parties, overriding any previous agreements, interpretations, or assertions.

26. RELEASE, CERTIFICATION & LIABILITY
Liability Release: I/We agree to release and safeguard the removal personnel, crematory, agents, and employees of Angel Paws USA from any claims, liabilities, costs, or expenses. Liability Cap: Angel Paws USA's liability for any negligent actions is limited to a reimbursement of $50. Certification: All statements and information provided in this form are confirmed to be accurate and truthful. I/We have read and understood this document in its entirety. Acknowledgment: By signing, expressing agreement, or submitting this Agreement, all Parties acknowledge their understanding and acceptance of the outlined terms and conditions.
