1. Owner Certification

Ownership: I certify that I am the owner or the authorized agent of the deceased pet.
Disposition Rights: I confirm that I possess the legal right to control the disposition of the remains of the pet.

2. Service Authorization

General Authorization: I/We grant full permission to Angel Paws USA to perform all actions related to pet aftercare services, including transportation, cremation, euthanasia, burial, and any other services as per the selected package.

• Cremation: If the Cremation Service is chosen, I authorize the removal and cremation of the pet.

• Euthanasia: If the Euthanasia service is selected, I grant permission for the euthanasia procedure, followed by removal and cremation.

• Burial: For Burial services, I authorize the removal and burial of the pet.

• Autopsy/Necropsy: If an autopsy or necropsy is opted for, I authorize a post-mortem evaluation of the pet.

3. General Transport

Transport Permission: I allow Angel Paws USA and their partners to manage or assist in transporting the remains, including transport to/from my home, an autopsy facility, or a crematory.
Documentation: I commit to providing or assisting in obtaining necessary documentation to thoroughly investigate the cause of death.

4. Partners Program Authorization

Authorization: For services associated with the Partners Program, I authorize cremation and/or removal as needed.

5. Rabies Exposure Declaration

Exposure Status: To my knowledge, the pet has not bitten, scratched, or potentially exposed any individual or entity to rabies within the past ten (10) days.
Rabies Test: If the pet has bitten or may have exposed someone within the specified timeframe, a rabies test is required. We will not handle such cases as the head must be sent to a lab.

6. Euthanasia Awareness

Understanding: Euthanasia is acknowledged as a compassionate measure to end an animal's life and alleviate suffering.
Accuracy: I guarantee all provided information is accurate and comprehensive.
Immediate Execution: By signing this agreement, I understand my stated wishes may be carried out promptly.
Process: The euthanasia process involves a sedative, anesthesia, and finally, a drug to stop the heart.

7. Transportation & Vehicle Selection

General Understanding: Angel Paws USA has the discretion to choose the vehicle type (cars, vans, SUVs, or pickups) for transporting my pet based on logistical needs.
Multiple Pet Transport: Angel Paws USA may transport multiple pets simultaneously to optimize efficiency. All pets will be treated with dignity and care.
Promptness Commitment: Angel Paws USA commits to reaching the pick-up location within an estimated 1-3 hours.

8. Variable Response Time

General Understanding: The processing time for removals may vary due to factors like call volumes and unforeseen circumstances. While we aim to be prompt, wait times may exceed 1-3 hours. Clients will be informed of expected response times, and refunds are offered if pickup can't occur within the same evening.

9. Crematory Selection

Role Clarity: Angel Paws USA operates as a pet aftercare service provider and has the authority to select the crematory for my pet.
Quality Assurance: All chosen crematories are evaluated to ensure they meet Angel Paws USA’s standards of care.

10. Payment Agreement & Terms Disclosure

Cremation Services Payment: A non-refundable deposit of $150 is required upon completing this form. Any remaining balance, including additional selections, will be charged after removal.
Euthanasia Services Payment: A non-refundable fee of $350 is required upon form completion. The remaining balance will be charged when the attending doctor arrives.
Necropsy/Autopsy Services Payment: Upfront payment covers transport, detailed examination, testing, pathologist consultation, and private cremation.
Partners Program: Charges are based on the pricing agreement provided.

11. Package Details & Timelines

Package Contents: Angel Paws USA reserves the right to modify package components. Items may be replaced or altered, especially if unavailable.
Estimated Package Ready Time: Packages are typically prepared within 7-10 business days (excluding weekends and holidays).
Unclaimed Packages: A storage charge of $50 applies if a package is unclaimed 15 days after readiness. Packages not claimed within 30 days will be discarded without reimbursement.

12. Dispute Resolution

Initial Resolution Steps: Parties should first attempt to resolve disagreements through open and honest discussions.
Mediation: If unresolved within a month, either party may initiate mediation. A mutually selected mediator will be chosen, or a reputable mediation organization will be sought.
Legal Action: If mediation is unsuccessful, legal remedies may be pursued exclusively in the courts of Miami-Dade County, with both parties waiving any objections regarding the venue.

13. Intellectual Property

Rights and Protections: All content, trademarks, logos, and other intellectual properties associated with Angel Paws USA are protected by copyright and trademark laws. Unauthorized use or replication without explicit consent is prohibited.

14. User-Generated Contributions

Usage Rights: Reviews, testimonials, or photos submitted may be used by Angel Paws USA for promotional purposes. Contributors retain rights to their submissions.

15. Prohibited Activities

Customer Conduct: Clients are expected to act respectfully and understandingly. Disruptive or harmful behavior may result in service termination.

16. Termination of Service

Service Refusal: Angel Paws USA reserves the right to refuse or terminate services for non-payment, disruptive behavior, or non-compliance with terms.

17. Warranties & Guarantees

Service Standards: Angel Paws USA adheres to industry-leading standards. Specific quality guarantees will be communicated as appropriate.

18. Electronic Communications

Communication Methods: By using our services, clients consent to receive communications via email, SMS, or other electronic means.

19. Third-Party Links or Services

External Referrals: Angel Paws USA may suggest third-party services or products. We are not responsible for the content or quality of these offerings.

20. Refund or Redressal Process

Service Concerns: All sales are final. Any grievances or concerns about services can be directed to customer care for consideration.

21. Data Protection, Privacy, & Client Confidentiality

Personal Data Handling: Angel Paws USA respects client privacy and handles personal data in accordance with data protection laws.
Usage: Personal data is used solely for facilitating requested services. No third-party disclosure occurs unless the client consents or it is legally required.
Client Confidentiality: Angel Paws USA is committed to maintaining the confidentiality of all client information, including personal and medical data.

22. Commitment to Empathy and Care

Client-Centered Approach: Angel Paws USA is committed to providing services with empathy and understanding, ensuring clients feel supported during challenging times.

23. Agreement Terms & Jurisdiction

Governing Principles: This Agreement is governed by the laws of the State of Florida, disregarding any conflict of law rules.
Complete Agreement: This document represents the entire agreement between the Parties, superseding any prior agreements or understandings.

24. Release, Certification, & Liability

Liability Release: I/We agree to release and indemnify the removal personnel, crematory, agents, and employees of Angel Paws USA from any claims or liabilities.
Liability Cap: Angel Paws USA’s liability for any negligent actions is capped at $50.
Certification: All provided statements are accurate and truthful. By signing, all Parties acknowledge their understanding and acceptance of these terms.
