• I certify that I am the legal owner or authorized agent of the pet referenced in this agreement.
• I affirm that I have the legal authority to authorize the disposition of the pet’s remains.

• I authorize Angel Paws USA to arrange and perform services selected by me, including cremation, euthanasia coordination, burial, transport, or related aftercare services.
• For euthanasia services, I understand Angel Paws USA coordinates care with licensed veterinary professionals.
• I acknowledge that burial and necropsy services may involve third-party facilities selected by Angel Paws USA.

• I authorize Angel Paws USA and its partners to transport my pet’s remains between locations as required to complete the selected service.
• I agree to provide accurate information and documentation necessary to complete services without delay.

Services initiated through veterinary or partner facilities are authorized for removal, transport, and cremation as applicable.

• To my knowledge, the pet has not exposed any person or animal to rabies within the past ten (10) days.
• I understand that pets requiring rabies testing cannot be accepted for cremation services.

• I understand euthanasia is a humane medical procedure intended to prevent suffering.
• I acknowledge that medical procedures are performed by licensed veterinarians and outcomes cannot be guaranteed.
• I confirm all information provided is accurate to the best of my knowledge.

• Angel Paws USA determines transport methods and vehicles based on logistics, safety, and pet size.
• Multiple pets may be transported simultaneously in accordance with professional standards.
• Estimated arrival times apply to removal services only and are not guarantees.

Response times may vary based on call volume, weather, availability, or other factors. Same-day service is not guaranteed unless confirmed by Angel Paws USA. No refunds apply when response times fall within communicated expectations.

Angel Paws USA selects crematories that meet internal quality and care standards. Crematory selection is at the discretion of Angel Paws USA.

Deposits are non-refundable. Final charges may include surcharges based on timing, weight, holidays, or additional services. Failure to complete payment may result in suspension of services.

• Completion times are estimates and not guaranteed delivery dates.
• Package contents may be substituted if items become unavailable.
• Unclaimed items may incur storage fees or disposal after notice.

Disputes will first be addressed through good-faith discussion. Legal proceedings fall under Miami-Dade County jurisdiction.

All branding, content, and materials are the property of Angel Paws USA and may not be reproduced without permission.

Submitted testimonials or media may be used for promotional purposes.

Abusive, threatening, or disruptive behavior may result in refusal of service.

Angel Paws USA reserves the right to terminate services for nonpayment or policy violations.

Services are provided with care and professionalism. No outcome guarantees are expressed or implied.

By using our services, you consent to communication via phone, SMS, or email.

Angel Paws USA is not responsible for services provided by external partners.

All sales are final. Concerns must be reported promptly for review.

Client information is handled confidentially and in compliance with applicable laws.

Angel Paws USA is committed to compassion, respect, and dignity in every service.

This agreement is governed by Florida law and represents the entire understanding between parties.

Liability is limited as permitted by law. Submission of this agreement constitutes acceptance of all terms.
